Its quite surprising how different EU countries have different treatment guidelines for the same product to treat the same condition..namely PA.

In the netherland the official recommendations for treating PA (Farmacotherapeutisch Kompas) are:

to have 10 injections à 1000 microg for loading with 3 days interval then maintenance 1000 microg once every two months (same hydroxocobalamin as used in UK), if no neurological involvement.

If neurological involvement the recommended treatment is:
1000 microg once - à twice per week for ie. 2 years.

This is the dutch version of leaflet that comes with hydroxocobalamin, make:Altana (its in dutch, perhaps some one here is able to use a google translate or something? )

Basically it recommends with my simple translation:

Hydrocobamine 1000 microgram, oplossing voor injectie 1000 microgram/2 ml.
1000 microgram in ampul 1 mg hydroxocobalamine hydrochloride per 2 ml liquid for injection (0,5 mg/ml).

Treatment:
The injection should be intramuscular of subcutanious.
Initially 10 injectiions à 1000 microgram Hydrocobamine with a min of three days inbetween.
Maintenance: 1000 microgram Hydrocobamine once per two months.
If neurological problems:
"Bij duidelijke neurologische afwijkingen 1000 microgram Hydrocobamine één à tweemaal per week gedurende geruime tijd, bv. 2 jaar." simple translation:
With neurological involvement: 1000 microgram Hydrocobamine once à twice per week for a long time, ie. 2 years.

"How Supplied

Nascobal Nasal Spray is available as a spray in 3 mL glass bottles containing 2.3 mL of solution. It is available in a dosage strength of 500 mcg per actuation (0.1 mL/actuation). (stuff about how the pump on the bottle works) One bottle will deliver 8 doses.


Information For Patients

Patients with pernicious anemia should be instructed that they will require weekly intranasal administration of Nascobal Nasal Spray for the remainder of their lives. Failure to do so will result in return of the anemia an in development of incapacitating and irreversible damage to the nerves of the spinal cord. Also, patients should be warned about the danger of taking folic acid in place of vitamin B12 because the former may prevent anemia but allow progression of subacute combined degeneration of the spinal cord..."

(stuff about hot foods and absorption of nasal products; stuff about diets and pregnancy)

Because the nasal dosage forms of Vitamin B12 have a lower absorption rate than intramuscular dosage, nasal dosage forms are administered weekly, rather than the monthly intramuscular dosage...(stuff about pictures demonstrating) The patient should also understand the importance of returning for follow-up blood tests every 3-6 months to confirm adequacy of the therapy."


I hope this is the kind of info you wanted. I finally went to the website and most of the leaflet is there.

Oh and I forgot to add this, but in the box with the bottle I get a set of stickers to put on a calender each time I use a dose--marking when my last one was. Very thoughtful of them, considering PA patients might have memory issues.
