Spurious Elevations of Vitamin B12 with Pernicious Anaemia

I have recently been passed this article "Spurious Elevations of Vitamin B12 with Pernicious Anaemia" which is by David T Yang and Rachel J Cook of University of Wisconsin School of Medicine and Public Health which was published in the New England Journal of Medicine. It highlights two women who presented with peripheral neuropathy and associated pancytopenia with macrocytic anemia but whose B12 levels were 1228 pmol per liter and 975 pmol per liter.

They went onto say that there have been false reports for vitamin B12 levels generated by automated analyzers and it was suggested that intrinsic factor antibodies may interfere with the assay. They went on to investigate further by adding 25% polyethylene glycol with the serum and discovered the actual vitamin B12 levels decreased to below the limit of detection.

I think it would be a very good idea for the Pernicious Anaemia Society to run a survey on how many people with PA actually test positive for intrinsic factor antibodies. It would also be a very good idea to run tests in a "normal" population to see how many people produce intrinsic factor antibodies in total.

This is definite proof that the serum B12 test machines used are unreliable, and I for one am living proof of that and it is time something was done to help people to gain access to enough vitamin B12.
